2. Survey Questionnaire of Personal Performance during and after the earthquake
　
　The survey was implemented Radiation Protection Supervisors in the areas of seismic intensity of greater than 5. The survey investigated the personal performances of the radiation protection supervisors during and after the earthquake.

　The Earthquake Survey Commission received replies from radiation protection supervisors of 540 radiation using facilities (64.2% reply rate).
　The CRPS issued the SRIC to facilities in the areas of specified city, towns and villages covered by the Disaster Relief Law. The reason why it was difficult to define the area of seismic intensity of greater than 5, because the disaster by this earthquake occurred in limited areas.
　The areas specified by the Disaster Relief Law are as follows:
1) In Hyogo Prefecture: Kobe, Amagasaki, Nishinomiya, Ashiya, Itami, Takarazuka, Kawanishi, Akashi, Miki, and Sumoto city, and Tsuna town, Awaji town, Hokudan town, Ichinomiya town, Higashiura town, Goshiki town, in Tsuna county, Seidan town, Mihara town, Midori town, and Nandan town in Mihara county.
2) In Osaka-fu: Toyonaka, Osaka, Ikeda, Suita, and Minoo city. There are 331 facilities in the area specified by the Disaster Relief Law, and survey covered 229 facilities (69.2%).
